PDS Express is an independent web app for preparing CSC Form 212, the Personal Data Sheet used in many Philippine government hiring and HR workflows.
It was created by Leigh Dinaya, a web developer and former government employee. The product comes from a simple frustration: PDS records change over time, but people often have to keep editing and rechecking scattered copies of the same form.
The goal is practical: keep one reusable PDS profile, preview the generated PDF, and export a file when you need it for an application, personnel update, promotion packet, renewal, or HR request.
Independent tool
PDS Express is not affiliated with, endorsed by, or connected to the Civil Service Commission or any government agency. Always follow the instructions from the office requesting your form and review every generated PDF before submission.
