Quick reference for students, graduates, and professionals preparing the Personal Data Sheet (CSC Form 212) for government applications, appointments, promotions, contract renewals, employee record updates, and personnel file compliance.
Account and sign-in
You do not need an account to fill, preview, or download the basic PDS PDF. Accounts are only needed for Pro Pass cloud saving. Sign-in is handled through Clerk; use Clerk's recovery flows from the sign-in page if you lose access.
Saving your PDS
Work through each section in the app. Free drafts stay in this browser through local storage when available. Pro Pass lets you use Save Draft or Verify & Save to persist your profile to our database so you can continue later from your account.
Preview and PDF export
The Review PDF area generates a preview from your current data. Download export is included in the free workflow and uses the data currently in your draft. Generated PDFs follow the CSC Form 212 field layout for applications, HR updates, and personnel files. Always review the file yourself before submitting it to any agency or HR office.
Resume import (optional)
You may upload a PDF resume to suggest values for some fields. Text is extracted from the PDF and processed with Google Gemini to map into PDS-shaped data. Results are imperfect—treat them as a starting point and correct every field against your official records.
