How To Fill Out CSC Form 212: A Practical PDS Guide

CSC Form 212, also called the Personal Data Sheet or PDS, can feel intimidating because it asks for many details at once. The easier way to handle it is to treat it as a profile you build section by section.

This guide is for preparation only. Always follow the instructions from the agency, school, employer, or HR office requesting your form.

Start With Your Personal Information

Begin with the fields you can verify quickly:

  • Full name
  • Date and place of birth
  • Citizenship
  • Civil status
  • Contact details
  • Residential and permanent addresses

Check spelling carefully. Names and addresses are easy to type quickly but annoying to correct later.

Prepare Your Identification Numbers

Only fill in the identification numbers that apply to you. Do not guess.

Common fields include:

  • GSIS
  • Pag-IBIG
  • PhilHealth
  • SSS
  • TIN
  • Agency employee number

Because these fields are sensitive, avoid sharing a completed PDS in public chats, unsecured folders, or random upload tools.

Add Education Records In Order

Education entries should match your supporting documents where possible.

For each level, check:

  • School name
  • Degree or course
  • Inclusive years
  • Highest level or units earned
  • Year graduated
  • Honors or academic distinctions

If a section does not apply, follow the instructions of the office requesting the form.

Check Eligibility And License Details

Eligibility fields matter for many government roles. Enter the details exactly as they appear on your record or certificate.

Review:

  • Eligibility or license name
  • Rating
  • Date of examination or conferment
  • Place of examination or conferment
  • License number and validity, if applicable

Review Work Experience Slowly

Work experience often creates the most mistakes because dates, appointment status, and agency names can vary across records.

For each entry, check:

  • Inclusive dates
  • Position title
  • Department, agency, office, or company
  • Monthly salary or salary grade
  • Status of appointment
  • Whether it is government service

If a role title is long, preview the PDF to make sure it remains readable.

Add Trainings And Seminars

Training records can stack up over time. Record the exact title, dates, hours, type, and sponsor or organizer.

The official form has limited space, so prioritize entries that are relevant to the requirement you are preparing for.

Do A Final Preview

Before you submit or print, preview the PDF and look for:

  • Missing required fields
  • Wrong dates
  • Cut-off text
  • Duplicate entries
  • Old contact details
